Please complete the Expression of Interest form below to commence the credentialing process. You will then receive an email inviting you to credential with Dubbo Private Hospital and requesting full credentialing information.

Please note, these documents must be included in your credentialing application:
- Current curriculum vitae
- Evidence of current AHPRA registration
- Specialist qualification or fellowship certificate, if applicable
- Professional indemnity insurance certificate of currency
- Proof of identity (100 points)
- Current Police Check
- Working with Children Check (per state requirements)
- Immunisation evidence
- Names and contact details for 3 professional referees (one referee must be practicing in the same specialty and have recent knowledge of the applicant)
- Radiation Licence (if applicable)
